Before registering for the class, please read and acknowledge the following:

FIREARMS TRAINING WAIVER & LIABILITY RELEASE

ACKNOWLEDGMENT OF RISK

I, the undersigned, acknowledge and fully understand that participation in any firearms training, including but not limited to live-fire shooting exercises, carries inherent risks. These risks include, but are not limited to:

Serious bodily injury or death
Hearing damage
Ricochets, negligent discharges, and equipment failure
Environmental hazards (weather, terrain, hot brass, etc.)
Psychological trauma and/or other personal or financial liability

I voluntarily agree to assume full responsibility for any and all risks of injury, death, psychological impact, or property damage resulting from participation in this course.

RELEASE OF LIABILITY

By signing this document, I release and hold harmless the following parties from any and all liability or claims for injury, death, or damages:

Instructor Thai Nguyen
Any assistant instructors or range staff
The range facility and its owners/operators
USCCA or any affiliated partners

I understand that Instructor Thai Nguyen is not liable for any incidents that occur on the range or thereafter, including but not limited to:

Accidental or negligent discharges
Improper firearm handling or usage
Emotional, legal, or physical consequences resulting from my use of a firearm after class completion

STUDENT RESPONSIBILITIES

I agree to:

Follow all range rules and instructor commands without exception
Maintain proper muzzle and trigger discipline
Use only authorized firearms and ammunition
Wear eye and ear protection at all times
Notify the instructor immediately of any unsafe condition or incident

Failure to comply with these rules will result in immediate removal from the range with no refund.

NO GUARANTEE OR WARRANTY

I understand that successful completion of this course does not guarantee my proficiency, judgment, or legal protection in any real-world self-defense encounter. The instruction provided is for educational purposes only. Any misuse of knowledge or tools taught in this course is my sole responsibility.

LEGAL DISCLAIMER

This waiver is legally binding. By signing below, I confirm that:

I am legally allowed to handle and operate a firearm
I am mentally and physically fit to participate in firearms training
I have read and fully understand this agreement
I sign this release voluntarily and without coercion

Criminal History Disclosure
If you are currently under indictment, facing legal restrictions, or have any criminal history that may disqualify you from possessing or using a firearm under federal or state law, you are required to disclose this information to the instructor before class. Failing to do so may result in immediate removal from the course and potential legal consequences.

STUDENT PRIVACY NOTICE

CONFIDENTIALITY OF STUDENT INFORMATION

I, the undersigned, understand that any personal information I provide during the registration or participation in this firearms training course will be treated with strict confidentiality.

This includes, but is not limited to:

Full name, email, phone number, and address
Emergency contact information
Firearm ownership or usage details
Class registration details submitted via online or paper forms

I acknowledge and agree that:

My information will be used only for training-related purposes, including communication, certification, liability documentation, and safety compliance.
My data will not be sold, disclosed, or shared with third parties outside of Instructor Thai Nguyen’s direct training programs, affiliated partners (e.g., USCCA), or legal authorities if required by law.
I may receive class-related reminders, updates, or follow-up resources through email or text based on the information I provide.

By signing this document, I permit Instructor Thai Nguyen to store my information for official class use only securely. I understand that I may request the removal of my data from non-essential mailing lists at any time.

COURSE REGISTRATION & USCCA ACKNOWLEDGMENT

REGISTRATION AGREEMENT

By submitting this form and participating in this course, I acknowledge that I am registering for a firearms training class provided by Instructor Thai Nguyen and affiliated with the United States Concealed Carry Association (USCCA) when applicable.

I understand and agree to the following:

This submission serves as a formal commitment to attend the selected class date and location provided by Instructor Thai Nguyen.
My seat in the course is reserved upon submission of this completed form, and I am expected to attend and participate in full unless otherwise arranged in advance.
I may be required to complete additional enrollment steps through the official USCCA platform or class portal prior to receiving certification, depending on course type.
My participation in this course may be documented through USCCA channels for training records, certificates, or class verification purposes.

INSTRUCTOR DISCLOSURE

Instructor Thai Nguyen is a certified USCCA Firearms Instructor. While some classes are conducted under USCCA guidelines, certain Texas License to Carry (LTC) sessions may operate under state-specific requirements and standards outside of USCCA jurisdiction.

By signing below, I confirm that I understand and agree that this course may be certified through the USCCA or Texas Department of Public Safety depending on its structure and requirements.

PAYMENT ACKNOWLEDGMENT & POLICY

COURSE PAYMENT POLICY

I acknowledge that payment for this course must be completed within ten (10) days of class unless otherwise approved by Instructor Thai Nguyen in writing.

I understand and agree to the following:

My seat in the class is not fully confirmed until payment is received in full.
Payment may be accepted via Zelle, Venmo, PayPal, the USCCA Website, or other methods approved in advance.
Failure to submit payment by the scheduled class date may result in removal from the course without certification or documentation of attendance.
Refunds or reschedules must be requested at least 72 hours before the scheduled class time. No-shows or last-minute cancellations may result in forfeiture of the registration fee.

I understand that Instructor Thai Nguyen reserves the right to approve or deny refunds and reschedules on a case-by-case basis.

STUDENT RESPONSIBILITIES & BEHAVIOR POLICY

PARTICIPANT CODE OF CONDUCT

I agree to conduct myself in a safe, respectful, and professional manner at all times throughout the duration of the course. I further agree to the following responsibilities:

Arrive on time and prepared for the classroom and/or range portion of the course.
Follow all safety instructions, range commands, and class rules without hesitation or argument.
Treat all fellow students, instructors, and range staff with respect and professionalism.
Use safe handling practices for firearms, ammunition, and related equipment at all times.
Immediately notify the instructor of any unsafe behavior, incident, or medical concern.

UNSAFE OR DISRUPTIVE BEHAVIOR POLICY

I understand that failure to comply with safety rules, instructor commands, or class expectations may result in my immediate removal from the course without refund. This includes, but is not limited to:

Horseplay or distractions during range instruction
Disregard for safety rules
Aggressive or disrespectful behavior
Handling firearms without permission or unsafe muzzle/trigger discipline

POLITICAL CONDUCT POLICY

I understand that maintaining a safe, respectful, and focused learning environment is critical for the success of all students and instructors. Therefore, the following rules regarding political conduct are strictly enforced:

No Political Discussions: Politics should not be discussed during class sessions or any instructional activities.
No Harassment: Harassing, confronting, or otherwise pressuring other students or instructors about political beliefs or opinions is strictly prohibited.
Immediate Removal: Failure to comply with this policy will result in immediate removal from the course and expulsion from the premises.
No Refunds: Students asked to leave due to violations of this policy will not receive a refund for any portion of the course fees.
Respectful Environment: All students must maintain an atmosphere of mutual respect and refrain from comments or behaviors that may provoke, demean, or intimidate others.
Reporting Misconduct: Students who experience or witness political harassment or misconduct are encouraged to report it promptly to the instructor.

REFUND & RESCHEDULING POLICY

CANCELLATIONS AND RESCHEDULES

I understand that my class fee secures a limited seat, and Instructor Thai Nguyen incurs costs and preparations for each registered student.

Therefore:

I may cancel or reschedule my class without penalty if I notify the instructor at least 72 hours prior to the class start time.
Cancellations or rescheduling requests within 72 hours may be denied or result in a partial refund at the discretion of the instructor.
No-shows without prior communication will not receive a refund and may be denied re-enrollment.
In the event of an emergency, I agree to contact Instructor Thai Nguyen as soon as possible to request rescheduling or to discuss alternative options.
